Can I collect patient-identifiable information for a certified CME activity? Does this information need to be included in the activity file for audit by the ACCME?

No, the ACCME will not allow an accredited provider to forward to them any patient-identifiable information for the purposes of accreditation.  And, as a provider, you cannot keep as a part of your records of continuing medical education patient identifiable information.  That’s confidential information and ACCME (along with the accredited provider) shouldn’t have it and doesn’t need it!
