Does a completed disclosure form demonstrate conflict resolution?

No.  Collection of disclosure information via a signed disclosure form is a method of identification of relevant COIs.  Conflict resolution occurs when an appropriate resolution mechanism is identified and implemented, once disclosure information has been collected.
