For any learners wishing to claim MOC for an activity, a provider needs to first obtain permission from the learner to share the information with the ACCME (so it can be shared with their board). The information required for collection is:
- Date of completion
- Learner’s certifying board ID (optional ABS)
- First name
- Last name
- Date of birth (month/day only)
- Which activity the learner completed
- Credit type(s)
- Quantity of MOC points
