What is the “interprofessional team” that’s described in Commendation Criteria 23?

The ACCME defines “interprofessional team” as a team that is made up of members from two (or more) different professions who learn with each other, from each other, and about each other in order to enable more effective collaborations and health outcome improvements.  The different professions cannot simply be different types of physicians, however; the professions should span different fields, such as nurses, physicians, community health workers, social workers, psychologists, pharmacists, etc.
