The phrase “professional practice gap” is used in multiple places – what does this mean?

The ACCME has adapted this definition from the Agency for Healthcare and Research and Quality (AHRQ).  It is defined as a “gap in the quality of patient care where the gap is the difference between health care processes or outcomes observed in practice, and those potentially achievable on the basis of current professional knowledge”.
