We’d like to demonstrate “healthcare quality improvement” for the Improves Healthcare Quality Criterion; how exactly do we do that?

The ACCME states that “healthcare quality improvement refers to improvements in clinical care processes or systems”.  As such, you, as the healthcare provider, are expected to both describe and provide data demonstrating the exact healthcare quality improvement you achieved (with the support of collaboration).
