What would “patient or community health” consist of for C38?

The ACCME explains that patient or community health are the “health characteristics or outcomes related to individuals within a geographic location, service area, or other grouping”.  Outcomes can include prevalence and/or incidence of disease, vaccinations, mortality, nutrition, social determinants, etc.
